В ретроспективное исследование включено 15 больных РР ММ с интактным и измеряемым PIg (G/κ, G/λ, А/κ). После завершения 6 противорецидивных циклов лечения бортезомибом оценку эффективности проводили по стандартным критериям и по анализу СЛЦ на анализаторе Hitachi 911. Использовали антитела к скрытым детерминантам СЛЦ (The Binding Site, Великобритания).</p></sec><sec><title>Результаты</title><p>Результаты. Частичный ответ (ЧО) был получен у 4, малый ответ у 5, стабилизация болезни у 6 больных. Не было случаев достижения полного ответа (ПО) или строгого полного ответа (СПО). На основании результатов анализа концентрации СЛЦ после лечения все больные были разделены на 2 группы с клональным или нормальным κ/λ-отношением. У 11 больных с ответом менее ЧО κ/λ-отношение носило клональный характер, что соответствовало концентрации интактного PIg. В то же время в 4 наблюдениях (ЧО) κ/λ-отношение СЛЦ было нормальным. Полученные результаты показали, что у 11 из 15 больных концентрация СЛЦ после курсов химиотерапии совпадала с концентрацией интактного PIg. У 4 из 15 больных такого совпадения не установлено. Вероятно, лечение бортезомибом наряду с противоопухолевым действием снижает активность генов, синтезирующих СЛЦ, что нормализует отношения легких и тяжелых цепей в опухолевой клетке. С учетом клинических данных это можно рассматривать как фактор благоприятного прогноза.</p></sec><sec><title>Заключение</title><p>Заключение. Метод определения СЛЦ можно иcпользовать при РР ММ с интактным измеряемым PIg в качестве оценки эффективности химиотерапии. Факты остановки синтеза опухолевых СЛЦ при ЧО у 4 из 15 больных следует учитывать при установлении СПО. Остановка синтеза опухолевых СЛЦ имеет благоприятный клинический прогноз.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the concentration of immunoglobulin free light chains (FLC) in comparison with that of intact measurable paraproteins (PIg) in patients with relapsed/resistant multiple myeloma (RR MM) undergoing treatment with bortezomib.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective study included 15 patients with RR MM with intact measurable PIg. Following 6 cycles of bortezomib treatment, an evaluation of the treatment efficacy was performed using standard criteria and by analysing serum FLC of immunoglobulins (sFLC).</p></sec><sec><title>Results</title><p>Results. A partial response (PR) and small response was achieved in 4 and 5 patients, respectively. The stabilization of the disease was observed in 6 patients. No cases of complete response (CR) or stringent complete response (SCR) were recorded. On the basis of the data on the concentration of sFLC after treatment, all patients were divided into 2 groups: those with an abnormal (clonal) and normal κ/λ ratio. In 11 patients with a response lower than PR, sFLC κ/λ ratio was of a clonal nature, which corresponded to changes in the concentrations of intact PIg during treatment. In 4 cases with PR, the residual tumour was determined by the presence of intact PIg within the 32–45 % range under, however, a normal sFLC κ/λ ratio.</p></sec><sec><title>Conclusion</title><p>Conclusion. Treatment with bortezomib affects all processes in MM with intact PIg, such as synthesis of FLC by tumour plasma cells, a decrease in the amount of circulating sFLC in blood and in the concentration of intact PIg.
