Болезнь Гоше (БГ) — это аутосомно-рецессивная лизосомная болезнь накопления, обусловленная дефицитом фермента глюкоцереброзидазы, который участвует в деградации гликосфинголипидов. Поражение костносуставной системы является одним из основных проявлений заболевания и наблюдается у 70–100 % больных. Редкость БГ, неспецифические и гетерогенные клинические проявления, а также сложная лучевая семиотика могут затруднять дифференциальную диагностику БГ с рядом других заболеваний.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: представить лучевую семиотику обратимых и необратимых изменений костно-суставной системы при БГ I типа и определить роль магнитно-резонансной томографии (МРТ) в диагностике и оценке степени тяжести поражения костно-суставной системы у больных БГ I типа.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ретроспективно проанализированы данные рентгенограмм и МР-томограмм бедренных костей с захватом тазобедренных и коленных суставов 86 больных с диагнозом БГ I типа. Исследования были выполнены до начала специфической терапии. Оценивалось специфическое вовлечение костного мозга в патологический процесс. Определялись изменения костей и суставов, параартикулярных структур и мягких тканей.</p></sec><sec><title>Результаты</title><p>Результаты. Показан спектр изменений костно-суставной системы у больных БГ I типа. Установлено, что при БГ поражение костей может носить обратимый и необратимый характер. МРТ является чувствительным методом диагностики обратимых изменений костной системы при БГ.</p></sec><sec><title>Заключение</title><p>Заключение. Лучевая семиотика поражений костно-суставной системы у больных БГ типичная и вместе с тем крайне вариабельная. МРТ является «золотым стандартом» современной диагностики поражения костей при БГ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Gaucher disease is an autosomal recessive lysosomal storage disorder caused by deficiency of the enzyme glucocerebrosidase, which is required for the degradation of glycosphingolipids. Skeletal involvement is one of the main manifestations of the disease and is present in 70–100 % of patients. However, the rarity of Gaucher disease and non-specific and heterogeneous nature of its symptoms and radiological signs may impede consideration of this disease in the differential diagnosis.</p></sec><sec><title>Aim</title><p>Aim. To describe the radiological signs of bone involvement in type I Gaucher disease and assess the role of MRI in the evaluation of the severity of the disease in newly diagnosed patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. X-ray and MRI data of 86 treatment-naïve Gaucher patients were included in the retrospective analysis. Bone marrow involvement, as well as bone and joint pathological changes, were evaluated.</p></sec><sec><title>Results</title><p>Results. Radiological signs of bone involvement in Gaucher patients were described and classified into 2 groups, i.e. reversible and irreversible. Such a classification is important in terms of the assessment of the overall disease severity. MRI has proven to be a sensitive method for detection and evaluation of the reversible bone changes in Gaucher patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. Although radiological manifestations of bone involvement in Gaucher patients are typical, they may significantly vary and be present in various combinations.
