У большинства больных инфекция, вызванная SARS-CoV-2 (COronaVIrus Disease 2019 — COVID-19), проявляется как неосложненное острое респираторное заболевание. Однако у онкогематологических больных часто новая коронавирусная инфекция протекает в тяжелой форме.</p><p>Цель — анализ организации и результатов медицинской деятельности гематологического стационара во время пандемии COVID-19.</p></sec><sec><title>Материалы исследования</title><p>Материалы исследования. С  20  апреля по  20  ноября 2020  г. в НМИЦ гематологии было госпитализировано 2130 больных, из них 920 больных поступили в обсервационное отделение. У 907 (98,5 %) больных на момент поступления был отрицательный результат исследования на выявление РНК возбудителя SARS-CoV-2, 13 больных были госпитализированы по витальным показаниям без результата исследования отделяемого из носоглотки. Распределение по нозологиям было следующим: 235 (25,5 %) больных лимфомами, 152 (16,5 %) — множественной миеломой, 131 (14,2 %) — острым лейкозом, 86 (9,3 %) — гемофилией, 35 (4 %) — болезнью Гоше, 17 (1,8 %) — макроглобулинемией Вальденстрема, 16 (1,7 %) — апластической анемией, 153 (16,6 %) — различными заболеваниями для хирургического вмешательства, 81 (8,8 %) — иными гематологическими заболеваниями, а также 14 доноров костного мозга (1,6 %).</p></sec><sec><title>Результаты</title><p>Результаты. Из 920 больных, поступивших в обсервационное отделение, 139 (15 %) были госпитализированы в тяжелом состоянии, 653 (71 %) — в состоянии средней степени тяжести. У 124 (13,5 %) больных на момент поступления отмечалась лихорадка. 809 больным была выполнена компьютерная томография (КТ) органов грудной клетки, и у 121 (15 %) из них были обнаружены свежие воспалительные изменения легких. Было выявлено 24 (2,6 %) больных, инфицированных SARS-CoV-2 (у 20 больных COVID-19 идентифицирован методом полимеразной цепной реакции (ПЦР), у 4 больных — по данным КТ-картины органов грудной клетки). У 10 больных позитивный результат был получен при плановом обследовании или при развитии лихорадки в профильном отделении. Таким образом, прицельное обследование на COVID-19 в виде двукратного выполнения ПЦР-исследования на SARS-CoV-2 и КТ органов грудной клетки позволило дополнительно выявить 2,6 % больных COVID-19, несмотря на отрицательный результат теста перед поступлением в клинику.</p></sec><sec><title>Выводы</title><p>Выводы. Организация обсервационного отделения позволила снизить вероятность внутрибольничного распространения инфекции, вызванной SARS-CoV-2, и продолжить проведение специфической терапии всем нуждающимся больным.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The SARS-CoV-2 infection (COronaVIrus Disease 2019, COVID-19) usually progresses uncomplicated in an acute respiratory form but causes frequent severe illness in oncohaematological patients.</p><p>Aim — analysis of the management and efficacy of medical aid at a haematology clinic during the COVID-19 pandemic.</p></sec><sec><title>Patients</title><p>Patients. The clinic admitted 2,130 patients in April 20 — November 20, 2020, with 920 of them triaged into observatory wards. At the time of admission, 907 (98.5 %) patients were SARS-CoV-2 RNA-negative, with 13 hospitalised without nasopharyngeal swab tests. Patient nosology: 235 (25.5 %) lymphoma, 152 (16.5 %) multiple myeloma, 131 (14.2 %) acute leucaemia, 86 (9.3 %) haemophilia, 35 (4 %) Gaucher’s disease, 17 (1.8 %) Waldenstrom macroglobulinemia, 16 (1.7 %) aplastic anaemia, 153 (16.6 %) various surgical diseases, 81 (8.8 %) other haematological diseases and 14 (1.6 %) were bone marrow donors.</p></sec><sec><title>Results</title><p>Results. Among the 920 patients admitted to the observatory, 139 (15 %) were severe and 653 (71 %) had a moderate condition. Fever was reported in 124 (13.5  %) patients at admission. Chest computed tomography (CT) was performed in 809 patients, with recent lung inflammation detected in 121 (15 %) cases. Twenty four (2.6 %) patients were revealed SARS-CoV-2-positive, with 20 diagnoses PCR-verified and four — in chest CT. Ten patients were diagnosed positive in routine examination or at a specialty unit as fever aggravated. Thus, a two-staged SARS-CoV-2 screening with PCR and chest CT allowed an extra 2.6 % detection of COVID-19 cases despite negative tests at admission.</p></sec><sec><title>Conclusion</title><p>Conclusion.
