Выживаемость больных после трансплантации аллогенных гемопоэтических стволовых клеток (аллоТГСК) от неродственного или гаплоидентичного родственного донора повышается, если при HLA-типировании донор и реципиент подобраны с разрешением, позволяющим выявлять различия в некодирующих регионах HLAгенов. Секвенирование следующего поколения (next-generation sequencing — NGS) — метод, позволяющий выявлять замены в некодирующих регионах HLA-генов.</p><p>Цель — анализ результатов типирования генов HLA методом NGS.</p></sec><sec><title>Материалы и  методы</title><p>Материалы и  методы. Методом NGS с  помощью наборов AllType (One Lambda, США) HLA-генотипированы 1056 образцов ДНК больных с показаниями к алло-ТГСК, их родственных доноров и доноров регистра. Параллельно одновременно HLA-типировали 96  образцов ДНК по 8  генам HLA (A/B/C/DRB1/DRB3/DRB4/DRB5/DQB1) с получением результатов в течение одной рабочей недели.</p></sec><sec><title>Результаты</title><p>Результаты. Для HLA-генов класса I результаты получены на уровне 4-го поля (отдельного HLA-аллеля). Для HLAгенов класса II результаты получены с разрешением на уровне от 2-го до 4-го полей (от высокого до аллельного). Впервые для русской популяции (657  доноров регистра) получены результаты на уровне 4-го  поля по HLAгенам класса I. Установлены наиболее высокочастотные HLA-аллели: для гена HLA-А это А*02:01:01:01 (26,9 %), для HLA-В — B*07:02:01:01 (12,5  %), для HLA-С — C*07:02:01:03 (12,6  %). Среди HLA-генов класса  II наиболее частые варианты: DRB1*07:01:01 (14,1 %), DRB3*02:01:01 (18,0 %), DRB4*01:03:01 (18,9 %), DRB5*01:01:01 (13,5 %), DQB1*03:01P (17,4 %).</p></sec><sec><title>Заключение</title><p>Заключение. Использование NGS для HLA-типирования позволило получить результаты на уровне аллельного и  высокого разрешения с  минимальным количеством неоднозначностей при параллельном секвенировании большого числа образцов. Внедренный метод выявляет генетические полиморфизмы не только в  экзонах генов HLA, но и в некодирующих областях и может использоваться для типирования больных с показаниями к алло-ТГСК, родственных доноров и неродственных доноров.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The patient survival after allogeneic haematopoietic stem cell transplantation (allo-HSCT) from an unrelated or related haploidentical donor is improved in a donor–recipient match resolution at the level of non-coding region identity of HLA genes. Next-generation sequencing (NGS) allows detection of point substitutions in HLA non-coding regions.</p><p>Aim — assessment of the NGS-based HLA-typing performance.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An NGS-based HLA-typing of 1,056 DNA samples from allo-HSCT recipients, their related and registry donors was performed with AllTypekit chemistry (OneLambda, USA). A parallel HLA-typing assay of 96 samples by 8 genes (A/B/C/DRB1/DRB3/DRB4/DRB5/DQB1) was accomplished within one working week.</p></sec><sec><title>Results</title><p>Results. HLA class I genes were typed at a 4-field (allelic), and HLA class II genes — 2–4-field (high to allelic) resolution. An allelic-resolution typing of HLA class I genes in a Russian population (657 registry donors) was conducted for the first time. The most frequent HLA alleles have been identified: А*02:01:01:01 in HLA-A (26.9 %), B*07:02:01:01 in HLA-B (12.5 %) and C*07:02:01:03 in HLA-C (12.6 %). The most frequent HLA class II variants were DRB1*07:01:01 (14.1 %), DRB3*02:01:01 (18.0 %), DRB4*01:03:01 (18.9  %), DRB5*01:01:01 (13.5  %), DQB1*03:01P (17.4  %).</p></sec><sec><title>Conclusion</title><p>Conclusion. An NGS-geared HLA-typing has yielded low-ambiguity allelic and high-level resolution results in a parallel sequencing assay with a large number of samples.
