Проанализированы результаты клинического обследования и лабораторных исследований крови и мочи 40 больных диффузной B-ККЛ (20 мужчин и 20 женщин), которым проведена терапия по программе mNHL-BFM-90 в Гематологическом научном центре (ГНЦ, Москва) в период с 2002 по 2009 г. В группу сравнения вошли 19 больных (8 мужчин и 11 женщин), получивших терапию по программе СНОР/R-СНОР в тот же период. Медиана срока наблюдения после окончания лечения 6 лет. Лабораторные исследования крови и мочи в обеих группах выполняли перед началом химиотерапии и в момент исследования, т.е. через 5 лет и более после окончания химиотерапии. Из 40 больных диффузной B-ККЛ основной группы признаки хронической болезни почек выявлены у 32 (80%) против 12 (63%) группы сравнения, из них у 2 (5%) отмечалось снижение суточного клиренса креатинина. Частота нефротоксичных осложнений терапии mNHL-BFM-90 была значимо выше таковой программы СНОР/R-СНОР. Проведение высокодозной химиотерапии у 10 (25%) больных осложнилось острой почечной недостаточностью (ОПН) (однако из всех 86 больных, которым было выполнено высокодозное лечение с 2002 по 2009 г. ОПН перенесли 12%), из них двум больным потребовалось выполнение гемодиализа в отличие от группы стандартной химиотерапии (р= 0,01). В отдаленном периоде наблюдений статистически значимых различий по нефротоксическим проявлениям в сравниваемых группах не получено.</p></abstract><trans-abstract xml:lang="en"><p>To evaluate late nephrotoxicity of modified program NHL-BFM-90 (mNHL-BFM-90) in adult patients with poor-prognosis of diffuse large B-cell lymphoma (DLBCL). The data of laboratory tests of blood and urine of 40 patients (20 men and 20 women) with DLBCL treated during 2002--2009 years with modified NHL-BFM-90 protocol were analyzed. Comparison group included 19 patients (8 men and 11 women) treated with CHOP or R-CHOP. The median observation period after chemotherapy was 6 years. Laboratory tests of blood and urine were performed before chemotherapy and in 5 years after treatment. Results. Chronic kidney diseases were found in 32 (80%) patients treated with mNHL-BFM-90 vs in 12 (63%) treated with CHOP/R-CHOP. Only 2 (5%) patients had decreased creatinine clearance. The frequency of nephrotoxic complications was significantly higher in patients treated with mNHL-BFM-90 than with CHOP/R-CHOP. Treatment with high-dose chemotherapy was complicated by acute renal failure in 10 (25%) patients, 2 (20%) patients required hemodialysis in comparison with patients treated with standard chemotherapy (p= 0.01).
