Система гемостаза играет ключевую роль в патогенезе сердечно-сосудистых заболеваний.</p><p>Цель — оценить изменения количественных и качественных характеристик металлопротеазы ADAMTS13 и антигена фактора фон Виллебранда (vWF:Ag) у больных острым инфарктом миокарда (ОИМ) и ишемическим инсультом (ИИ).</p></sec><sec><title>Больные и методы</title><p>Больные и методы. В исследование включено 110 больных в возрасте 42–95 лет (медиана возраста — 64 года), из них: 65 больных ОИМ в возрасте 42–81 года (медиана возраста — 56 лет), у которых при выполнении ангиографии обнаружен тромбоз коронарных артерий, а также 45 больных ИИ в возрасте 40–95 лет (медиана возраста — 70 лет) с тромбозом церебральных артерий. Всем больным определяли антиген ADAMTS13 (ADAMTS13:Ag), активность ADAMTS13 (ADAMTS13:AC), антитела к ADAMTS13 (ADAMTS13:AB) и vWF:Ag. Образцы крови получали при выполнении ангиографии (артериальная и венозная кровь) и через 24 и 120 ч после выполнения ангиографии (венозная кровь).</p></sec><sec><title>Результаты</title><p>Результаты. В группе больных ОИМ выявлено увеличение значений показателя vWF:Ag, выходящее за верхнюю границу референсного интервала, при этом у больных ИИ в начале исследования этот показатель был в пределах референсного интервала и статистически значимо ниже по сравнению с больными ОИМ, но увеличивался через 24 ч и достигал максимума через 120 ч. При анализе изменений показателя ADAMTS13:Ag установлено, что он был статистически значимо выше во всех точках исследования в группе больных ОИМ по сравнению с группой больных ИИ, однако значения данного показателя не выходили за пределы референсного интервала у больных обеих групп. В то же время показатель ADAMTS13:AC во всех точках исследования был статистически значимо выше у больных ИИ по сравнению с группой больных ОИМ, а показатель ADAMTS13: AB, наоборот, в группе больных ОИМ был статистически значимо выше во всех точках исследования при сравнении с таковым для группы больных ИИ.</p></sec><sec><title>Заключение</title><p>Заключение. При исследовании двух патогенетических моделей тромбообразования установлено отсутствие взаимосвязи между показателем vWF:Ag и количественными или качественными характеристиками ADAMTS13. Увеличение показателя ADAMTS13:AB при ОИМ может рассматриваться как причина снижения показателя ADAMTS13:AC у этих больных.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The hemostasis system plays a key role in the pathogenesis of cardiovascular diseases.</p><p>Aim — to evaluate changes in quantitative and qualitative characteristics of ADAMTS13 metalloprotease and von Willebrand factor an tigen (vWF:Ag) in patients with acute myocardial in farction (AMI) and is chemic stroke (IS).</p></sec><sec><title>Patients and methods</title><p>Patients and methods. The study in cluded 110 patients aged 42–95 years (median age — 64 years), of which: 65 patients with AMI aged 42–81 years (median age — 56 years) who had coronary artery thrombosis during an giography, as well as 45 patients with IS aged 40–95 years (median age — 70 years) with diagnosed cerebral artery thrombosis. All patients were tested for ADAMTS13 an tigen, ADAMTS13 activity, an tibodies to ADAMTS13 and vWF:Ag. Blood samples were obtained during an giography (arterial and venous blood) and 24 and 120 hours after an giography (venous blood).</p></sec><sec><title>Results</title><p>Results. In the group of patients with AMI, an in crease in the values of the vWF:Ag in dicator was revealed, which went beyond the upper limit of the reference in terval. In patients with IS, at the beginning of the study, this in dicator was within the reference in terval and was statistically signiﬁcantly lower compared to patients with AMI but in creased after 24 hours and peaked at 120 hours. When an alyzing changes in the ADAMTS13:Ag in dicator, it was found that it was statistically signiﬁcantly higher at all points of the study in the group of patients with AMI, compared with the group of patients with IS, however, the values of this in dicator did not go beyond the reference in terval in patients of both groups. At the same time, the ADAMTS13:AC in dex at all points of the study was statistically signiﬁcantly higher in patients with IS, compared with the group of patients with AMI, and the ADAMTS13:AB in dex. On the contrary, in the group of AMI patients, this in dicator was statistically signiﬁcantly higher in all points of the study when compared with that for the group of patients with IS.</p></sec><sec><title>Conclusion</title><p>Conclusion. The study of two pathogenetic models of thrombosis demonstrated the absence of a relationship between the vWF:Ag in dex and the quantitative or qualitative characteristics of the ADAMTS13 metalloprotease.
