Рассматриваются гипотеза «гипермутабельности» данного локуса за счет прямого взаимодействия с соседними последовательностями нуклеотидов, характерных для гаплотипа 46/1, и гипотеза «благоприятной почвы», обусловленной непрямыми влияниями других ассоциированных генов гаплотипа на склонность к провоспалительным реакциям. Ранее не проводились исследования частоты носительства гаплотипа 46/1 JAK2 в региональных, возрастных и гендерных выборках.</p><p>Цель — анализ степени ассоциации между гаплотипом 46/1 и мутацией V617F гена JAK2 с полом, возрастом и местом проживания больных, обследованных в связи с подозрением на хронические МПН.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включено 949 образцов ДНК больных с подозрением на МПН. В контрольную группу для оценки распространенности гаплотипа 46/1 среди людей, не имевших симптомов МПН, были включены пробы 150 добровольцев и доноров крови. Для всех образцов проводили определение гаплотипа 46/1 (rs10974944), мутации V617F в гене JAK2, мутации в гене CALR (тип 1: c.1092_1143del; L367fs*46, COSV57116546; тип 2: c.1154_1155insTTGTC; K385fs*47, COSV57116551) и в гене MPL (W515K, W515L) методом полимеразной цепной реакции в режиме реального времени.</p></sec><sec><title>Результаты</title><p>Результаты. Показано, что распределение сцепленных минорных аллелей гаплотипа 46/1 JAK2 ассоциировано с наличием клинически значимого уровня (&gt; 2 %) аллельной нагрузки мутации JAK2 V617F. Отношение шансов риска выявления V617F-позитивного статуса при носительстве данного варианта гаплотипа не зависит от основного места проживания больных и максимально выражено у мужчин до 50 лет, но не зависит от возраста обследованных женщин.</p></sec><sec><title>Заключение</title><p>Заключение. Ассоциация гаплотипа 46/1 JAK2 с наличием других драйверных мутаций МПН в генах CALR или MPL была менее выражена, но также статистически значима, что свидетельствует в пользу гипотезы «благоприятной почвы», а не «гипермутабельности» гена JAK2.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Haplotype JAK2 46/1 is associated with more frequent development of Ph-negative myeloproliferative neoplasms (MPN) and with an increased detection rate of the JAK2 V617F mutation. At the same time, the molecular mechanisms of such associations remain unclear. Previously, there were no studies of regional, age and gender aspects of the predictive value of carriage of the 46/1 JAK2 haplotype, which could assess this relationship in some observations.</p><p>Aim — to analyze the degree of association between 46/1 haplotype and the V617F mutation of the JAK2 gene depending on the sex, age, and place of residence of patients examined for suspected MPN.</p></sec><sec><title>Methods</title><p>Methods. The study included 949 DNA samples from patients with suspected MPN. Samples of 150 volunteers and blood donors were included in the control group. Haplotype 46/1 (rs10974944), V617F mutation in the JAK2 gene, mutations in the CALR gene (type 1: c.1092_1143del; L367fs*46, COSV57116546; type 2: c.1154_1155insTTGTC; K385fs*47, COSV57116551) and in the MPL gene (W515K, W515L) were determined for all samples using real-time polymerase chain reaction (PCR-RT).</p></sec><sec><title>Results</title><p>Results. The 46/1 JAK2 haplotype were shown to be associated with a clinically signiﬁcant level (&gt; 2 %) of the allelic burden of the JAK2 V617F mutation. The odds ratio of the risk of developing a V617F positive MPN when carrying this haplotype variant did not depend on the main place of residence of the patients and was found to be most pronounced in men under 50 years of age.
