Лимфома из клеток маргинальной зоны в ряде случаев может проявлять себя только на этапе гистологической трансформации в агрессивные лимфомы, в том числе в диффузную В-крупноклеточную лимфому (ДВККЛ). Цель — представить клиническое наблюдение трансформированной в ДВККЛ (тДВККЛ) на момент диагностики лимфомы из клеток маргинальной зоны.</p></sec><sec><title>Основные сведения</title><p>Основные сведения. Приведено клиническое наблюдение рецидива ДВККЛ образования подчелюстной области, которое было удалено полностью, больная находилась под наблюдением 11 лет без лечения. Во время рецидива было выявлено образование нижней доли правого легкого с SUV max до 19,27 размерами 81×43×88 мм. Выполнена атипичная резекция нижней доли правого легкого. По данным гистологического и иммуногистохимического исследований установлен диагноз «лимфома из клеток маргинальной зоны с трансформацией в ДВККЛ». При исследовании с помощью полимеразной цепной реакции доказана идентичность клона индолентной и возникшей агрессивной В-клеточной лимфомы. Проведена терапия по программе «R-CHOP+леналидомид», достигнута полная ремиссия заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Lymphoma from cells of the marginal zone in some cases can manifest itself only at the stage of histological transformation into aggressive lymphomas, including diffuse large B-cell lymphoma (DLBCL).</p></sec><sec><title>Aim</title><p>Aim: to present a clinical observation of lymphoma transformed at the time of diagnosis from marginal zone cells to DLBCL.</p></sec><sec><title>Main findings</title><p>Main findings. A case report of relapsed DLBCL in the submandibular area is presented, which was observed for 11 years without treatment. At the time of relapse, a 81×43×88 mm mass in the lower lobe of the right lung with SUV max up to 19.27 was identified. Atypical resection of the lower lobe of the right lung was performed. Based on histology and immunohistochemistry, a diagnosis of DLBCL transformed from marginal zone B-cell lymphoma was established, with PCR confirming the identity of the clone of indolent and aggressive B-cell lymphoma.
