Экстракорпоральный фотоферез (ЭКФ) является привлекательным методом лечения реакции «трансплантат против хозяина» (РТПХ) после трансплантации аллогенных гемопоэтических стволовых клеток при отсутствии эффекта системной иммуносупрессии. Однако использование ЭКФ ограничено необходимостью проведения лейкоцитафереза. Существуют модификации ЭКФ без применения лейкоцитафереза, в частности малообъемный ЭКФ (МО-ЭКФ).</p></sec><sec><title>Цель</title><p>Цель: анализ применения МО-ЭКФ для лечения больных с острой РТПХ (оРТПХ) и хронической РТПХ (хРТПХ).</p></sec><sec><title>Больные и методы</title><p>Больные и методы. Проанализированы результаты лечения 9 больных оРТПХ и 15 больных хРТПХ методом МОЭКФ за период с апреля 2021 по март 2024 г. Клеточный продукт был получен методом эксфузии 15–30 мл цельной крови с последующей фотохимиообработкой. Обработанный аутологичный клеточный продукт вводили больному.</p></sec><sec><title>Результаты</title><p>Результаты. Частота общего ответа у больных оРТПХ составила 44 %, а частота органоспецифического ответа при поражении кожи составила 56 %, при поражении желудочно-кишечного тракта (ЖКТ) — 40 %. При лечении хРТПХ частота общего ответа составила 73,3 %; эффективность терапии при поражении кожи — 71 %; при поражении ЖКТ — 67 %; при поражении легких — 67 %; при поражении глаз — 100 %, при поражении печени — 100 %.</p></sec><sec><title>Заключение</title><p>Заключение. МО-ЭКФ может рассматриваться как эффективный метод лечения оРТПХ и хРТПХ и как альтернатива стандартному ЭКФ при невозможности выполнения лейкоцитафереза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Extracorporeal photopheresis (ECP) is an attractive method of treating graft-versus-host disease (GVHD) after allogeneic hematopoietic stem cell transplantation due to the lack of systemic immunosuppression. However, the clinical use of ECP is often limited by the need for leukapheresis. There are modifications of ECP without the use of leukapheresis, in particular low-volume ECP.</p></sec><sec><title>Aim</title><p>Aim: to analyze the use of lv-ECP for the treatment of patients with acute and chronic GvHD.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. The results of treatment of 9 cases of acute GVHD and 15 cases of chronic GVHD using the lv-ECP from April 2021 to March 2024 are presented. The cellular product was obtained by effusion of 15-30 ml of whole blood followed by photochemical treatment. The processed autologous cell product was administered to the patient.</p></sec><sec><title>Results</title><p>Results. The overall response in patients with acute GVHD was 44%, organ-specific response for skin lesions was 56 %, and for gastrointestinal lesions, 40%.
