Представлено клиническое наблюдение за больной волосатоклеточным лейкозом (ВКЛ), у которой заболевание манифестировало двусторонней пневмонией на фоне опухолевого агранулоцитоза с развитием выраженной острой дыхательной недостаточности (ОДН), потребовавшей перевода больной на искусственную вентиляцию легких. Регресс ОДН и положительная динамика в течении пневмонии были достигнуты в результате спленэктомии, после которой нормализовалось число гранулоцитов крови, а также после назначения вориконазола. Проведение торакоскопической биопсии легкого не позволило выявить патогены, только после выполнения повторного бронхоальвеолярного лаважа удалось установить этиологическую причину поражения легких: грибы рода Aspergillus. Наличие мутации в гене BRAF V600E позволило начать терапию ингибитором BRAF-киназы вемурафенибом. В отличие от терапии цитостатическими препаратами это позволило избежать снижения лейкоцитов и прогрессию инфекционных осложнений. В последующем, через 4 мес у больной была достигнута клинико-гематологическая ремиссия заболевания.

The case report of the patient with hairy cell leukemia. The first manifestation of the disease was acute respiratory failure due to pneumonia. Initially, the clinical improvement was achieved due to splenectomy, which resulted in the increase of the blood granulocytic cells. A specific microbiological diagnosis was not established by thoracoscopy and lung biopsy. However after surgery bronchoalveolar lavage was revealed invasive aspergillosis. Successful treatment with voriconasole was started. BRAF-V600E mutation was searched out. After the clinical improvement the therapy with vemurafenib was started. This treatment helps to avoid further neutropenia and progression of infection complications. The clinical and hematological remission was achieved within 4 months.
