Системная химиотерапия (ХТ) значительно улучшила результаты лечения классической лимфомы Ходжкина (кЛХ), однако у 20–30% больных происходит рецидив заболевания, а в 10% случаев развивается рефрактерность к лечению. Выбор второй линии терапии у больных при рецидивах и рефрактерном течении (Р/Р) кЛХ зачастую зависит от опыта лечебного учреждения и врача.</p></sec><sec><title>Цель</title><p>Цель: оценить эффективность трансплантации аутологичных гемопоэтических стволовых клеток (ауто-ТГСК) и роль поддерживающей терапии ниволумабом после иммунохимиотерапии по протоколу «Nivo-BeGEV» при Р/Р кЛХ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. С 2019 г. по 2025 г. в проспективное клиническое исследование включено 134 больных с Р/Р кЛХ, получавших иммунохимиотерапию по протоколу «Nivo-BeGEV». Медиана возраста составила 32,5 (18–64) года: женщин — 63 (47%) и мужчин — 71 (53%). Ауто-ТГСК не была выполнена 19 (14,1%) больным, однако они получили поддерживающую терапию ниволумабом. Ауто-ТГСК была выполнена у 115 больных, из них 57 (49,6%) получили поддерживающую терапию ниволумабом, а у 58 (50,4%) больных она не была проведена.</p></sec><sec><title>Результаты</title><p>Результаты. В группе больных Р/Р кЛХ, у которых отсутствовал риск рецидива и прогрессии не было неблагоприятных событий. При выполнении протокола «Nivo-BEGEV» + ауто-ТГСК + поддерживающая терапия ниволумабом бессобытийная выживаемость (БСВ) на сроке 12 мес. составила 94%, на сроке 24 мес. — 89%, на сроке 36 мес. — 89%, а при проведении только ауто-ТГСК или только терапии ниволумабом —78% и 71%, соответственно (p =0,1617).</p></sec><sec><title>Заключение</title><p>Заключение. Показана значимость поддерживающей терапии ингибиторами контрольных точек в группе больных Р/Р кЛХ с высоким риском прогрессии и рецидивов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Systemic chemotherapy (CT) has significantly improved the treatment outcomes of classical Hodgkin lymphoma (cHL). However 20–30% of patients experience disease relapse and 10% develop refractory disease. The choice of secondline therapy for patients with relapsed or refractory (r/r) cHL often depends on the experience of the hospital and physician.</p></sec><sec><title>Aim</title><p>Aim: To evaluate the effectiveness of autologous hematopoietic stem cell transplantation (auto-HSCT) and the role maintenance therapy of nivolumab according to the Nivo-BeGEV protocol in r/r cHL.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From 2019 to 2025, 134 patients with r/r cHL who received immunochemotherapy according to the Nivo-BeGEV protocol were included in a prospective clinical study. The median age was 32.5 years (range 18–64), 63 (47%) were female and 71 (53%) male. Auto-HSCT was not performed in 19 (14.1%) patients, however they received maintenance therapy of nivolumab. Auto-HSCT was performed in 115 patients, of whom 57 (49.6%) received nivolumab maintenance therapy, while 58 (50.4%) did not.</p></sec><sec><title>Results</title><p>Results. In the group of r/r cHL patients without risk of relapse and progression, no adverse events were recorded. For patients treated with the Nivo-BEGEV protocol followed by auto-HSCT and nivolumab maintenance progression-free survival (PFS) rates were 94% at 12 months, 89% at 24 months and 89% at 36 months. Patients who underwent only auto-HSCT or only nivolumab maintenance had PFS rates of 88%, 78% and 71% respectively (p = 0.1617).</p></sec><sec><title>Conclusion</title><p>Conclusion.
