Развитие рефрактерности к терапии и рецидивов острого миелоидного лейкоза (ОМЛ), особенно в группе промежуточного прогноза, может быть обусловлено молекулярно-генетической гетерогенностью опухолевых клеток. Хромосомный микроматричный анализ (ХМА) позволяет выявлять микроделеции, дупликации и  копийнонейтральную потерю гетерозиготности (cnLOH), которые могут быть ассоциированы с ответом на терапию.</p></sec><sec><title>Цель</title><p>Цель: оценить частоту аберраций копийности и  cnLOH в  генах, ассоциированных с  лейкемогенезом, у  больных ОМЛ промежуточного прогноза и их связь с выживаемостью и ответом на лечение.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены 35 больных de novo ОМЛ из группы промежуточного прогноза по ELN‑2017. Анализ копийности по результатам ХМА проведен для панели из 36 генов, ассоциированных с лейкемогенезом. Референсная группа — 102 здоровых лица без онкогематологического анамнеза, которым также был выполнен ХМА.</p></sec><sec><title>Результаты</title><p>Результаты. Аберрации обнаружены у 91,18% больных ОМЛ, чаще всего в генах модификаторов хроматина (64,7% больных) и онкосупрессоров (64,7% больных). Преобладал тип cnLOH (PHF6, SMC1A, BCORL1). Дупликации KMT2A встречались только у больных ОМЛ — 14,3% (p &lt; 0,001) и ассоциированы с худшей выживаемостью (P лог-ранг = 0,05). Сочетания аберраций в 4–7 функциональных группах обнаружены у 20,6% больных.</p></sec><sec><title>Заключение</title><p>Заключение. Аберрации драйверных генов, особенно дупликация KMT2A, ассоциированы с неблагоприятным клиническим исходом при ОМЛ промежуточного прогноза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The development of therapy resistance and relapses of acute myeloid leukemia (AML), especially in the intermediate prognosis group, may be due to the molecular genetic heterogeneity of tumor cells. Chromosomal microarray analysis (CMA) can detect microdeletions, duplications, and copy-neutral loss of heterozygosity (cnLOH) which may be associated with a response to therapy.</p></sec><sec><title>Aim</title><p>Aim: to evaluate tthe frequency of copy number aberrations and cnLOH in leukemogenesis-associated genes in patients with intermediate-stage AML and and their relationship to survival and response to treatment.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 35 patients with de novo AML from the intermediate prognosis group for ELN2017. Copy number analysis by CMA was performed for a panel of 36 genes associated with leukemogenesis. The reference group included 102 healthy individuals without oncohematological disorders who also underwent comparable CMA testing.</p></sec><sec><title>Results</title><p>Results. Genomic aberrations were detected in 91.18 % of patients, most often in the genes of chromatin modifiers (64.7 % patients) and tumor suppressor genes (64.7% patients). The cnLOH type (PHF6, SMC1A, BKORL1) prevailed. KMT2A duplications occurred only in AML patients — 14.3 % (p &lt; 0.001) and were associated with worse survival (log-rank P = 0.05). Combinations of genomic alterations involving 4–7 functional gene groups were found in 20.6% of patients.</p></sec><sec><title>Conclusion</title><p>Conclusion.
