Хроническая реакция «трансплантат против хозяина» (хрРТПХ) является поздним осложнением трансплантации аллогенных гемопоэтических стволовых клеток (алло-ТГСК). Терапия хрРТПХ первой линии основана на применении глюкокортикоидов, однако у значительной доли больных формируется рефрактерное течение. В последние годы в клиническую практику внедрен селективный ингибитор ROCK2 белумосудил.</p></sec><sec><title>Цель</title><p>Цель: оценить эффективность и безопасность белумосудила у больных с тяжелой стероид-рефрактерной хрРТПХ.</p></sec><sec><title>Основные сведения</title><p>Основные сведения. Представлены клинические наблюдения больных с тяжелой хрРТПХ с поражением легких, резистентной к глюкокортикоидам и предшествующей терапии (включая ингибиторы JAK, антифибротические и иммуномодулирующие препараты). Белумосудил применяли в качестве 3-й и последующих линий лечения. В результате терапии отмечено достижение частичного ответа у одного больного и стабилизация заболевания у двух больных. Во всех случаях зарегистрирован удовлетворительный профиль безопасности, без развития серьезных нежелательных явлений. Полученные данные свидетельствуют о клинической эффективности белумосудила, включая больных с выраженным поражением легких. Ограниченная обратимость фиброзных изменений определяет целесообразность раннего назначения препарата.</p></sec></abstract><trans-abstract xml:lang="en"><p>Chronic graft-versus-host disease (cGVHD) is one of the most significant late complications of allogeneic hematopoietic stem cell transplantation (allo-HSCT). First-line therapy is based on glucocorticosteroids; however, a substantial proportion of patients develop steroid-refractory disease requiring subsequent lines of treatment. In recent years, targeted therapies have been introduced into clinical practice, including belumosudil, a selective ROCK2 inhibitor.</p><sec><title>Aim</title><p>Aim: to evaluate the efficacy and safety of belumosudil in patients with severe steroid-refractory cGVHD.</p></sec><sec><title>Main findings</title><p>Main findings. In the presented case series, patients with severe cGVHD with lung involvement, refractory to glucocorticosteroids and prior therapies (including JAK inhibitors, antifibrotic, and immunomodulatory agents), received belumosudil as thirdor later-line treatment. Partial response was achieved in one patient, while disease stabilization was observed in two patients. In all cases, the treatment demonstrated a favorable safety profile, with no serious adverse events requiring discontinuation. These findings suggest that belumosudil is clinically effective in heavily pretreated patients, including those with advanced pulmonary involvement.
